Kindergarten Application

 

OLPH School is accepting applications for our Fall 2012 Kindergarten class.  An information session was held on Monday, February 15th. Prospective families are welcome to meet with Mrs. Errera, our Kindergarten teacher, and visit the classroom during the school year, please contact the school office to schedule.
 
Parents of all prospective students must complete a Kindergarten Application (available for download below) and provide the following as part of the application process:
 
  • Birth certificate
  • Baptismal certification and proof of receipt of other sacraments depending on age (if Catholic)
  • Record of required immunizations

  • A $150 nonrefundable fee must accompany the forms in order to hold a place in Kindergarten. This fee is applied toward the total registration fee of $250.00 if child is offered enrollment.
